Informed Consent
- All patients will receive an email confirmation at the time of booking which includes a pre assessment and consent form which needs to be completed prior to attending for their appointment. This includes information on what to expect at your first session, terms and conditions of the clinic, chaperone procedures and information on informed decision making.
- Consent is taken verbally and recorded as part of the Cliniko notes at each treatment session. If a new treatment is being performed such as Acupuncture / Dry needling or attendance to a Pilates class, a separate consent form needs to be completed and if this is in paper format must be scanned and saved on to the client notes in Cliniko. The hard copy should then be shredded, and the scanned version deleted.
- Pilates consent forms will be saved under the individual client’s name on Acuity. It is the Pilates teacher’s responsibility to ensure they have checked and read this form prior to the client attending the class.
- We do see children under the age of 16 years old. We ask that a legal guardian is present to give informed consent for anyone under the age of 13 years old. We also encourage joint informed consent by the patient and legal guardian for anyone between 13-18 years of age.
- Legal guardians are asked to be present at all assessment and follow up treatment sessions. This can be re considered if the patient is between 13-16 years old and has deemed Gillick Competent. This will need to be discussed on an individual basis with the treating therapist and parties involved.
- If an adult patient attends for treatment and you feel they may not have capacity to give informed consent, it is important you rise your concerns with their GP before continuing any further treatment to decide best practice going forwards.
